About The Project

ProEmpower is a pre–commercial procurement (PCP) project to buy R&D (research and development) services to improve the treatment and self-management of diabetes type 2 patients using a Personalised Diabetes Management Solution.

Our consortium has been selected as one of the 5 tenderers for the phase 1 of the ProEmpower Project.

SHaDE – Smart Holistic Diabetes Ecosystem

The growing prevalence of diabetes represents a major challenge and requires to renew the current care models as shown by RE.MO.TE study inspiring SHaDE. SHaDE is an ecosystem aiming at meeting the procurers’ challenges with an ICT solution and a holistic (technological, clinical and organizational) approach for the DM2 IC management.

It allows health professionals to jointly support the patients and enables an active engagement of the latters, while teaching how to effectively manage the disease with learning tools, coaching support and communities.

The main benefits for procurers are to impact diabetes outcomes and to produce important results in terms of morbidity and resources utilization (e.g. number of admissions, visits per person/year).

Despite differences among countries (including the Procurers’ ones), SHaDE has been thought to be flexibly adapted, from both technical and organizational viewpoints.

A change management strategy is an essential driver to adopt health ICT initiatives as it can give many benefits to all stakeholders, overcoming their resistance to change and increasing their engagement. This is possible as specific SHaDE objectives are: (i) managing issues and expectations of stakeholders, (ii) developing a proper communication and disseminating knowledge and train on specific skills among them, (iii) establishing monitoring and evaluation processes for the whole project’s life cycle. The organization plan established to design, develop and introduce our solution in the procurers’ systems and processes has been conceived just on the basis of those three pillars, and it can count on a significant involvement of stakeholders in the form of advisory team, study group and procurers’ representatives.

The commercialization capacity of the consortium partners will allow to exploit the R&D results and to sustain a viable solution in healthcare market, even beyond the Procurers’ Countries.
